After you book an appointment online, via email, or by phone, you will receive an online form via email that asks about your health concerns, medications you are taking, medical history, insurance information, and basic contact info. A HIPAA-compliant confidentiality form will also be sent to you.
STARTING JUNE 21ST
We will be in-network with Aetna, MVP, BlueCrossBlueShield (Anthem and Highmark), CDPHP, NYSHIP, and United. Some insurance companies may provide out-of-network coverage for acupuncture. Our insurance biller will check if your benefit coverage allows for acupuncture visits. Please allow 2-3 days before your initial visit for us to confirm, starting June 21st. You can fill out the insurance information form sent to you for future reference.
Note: Standard government Medicare plans will NOT cover acupuncture.
A detailed intake will be taken where questions will be asked to review your health history and future goals. I will also perform a brief physical examination, including a tongue and pulse diagnosis and the corresponding meridians involved. This process will take about 30 minutes. A comprehensive treatment that addresses the body as a whole and also involves treatment for the main health concern will be done. This treatment session typically lasts 45–60 minutes. Aftercare guidance, realistic timelines, and options for ongoing treatments or referrals will also be given.
